Passport Renewal Requirements:

1. Personal appearance is required (except when the applicants are either 8 years old or younger, 65 years old and older and mentally or physically incapacitated).
2. Duly Accomplished Application Form. Download **
3. Four (4) colored photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photos must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Image of the applicant should occupy 70-80% of the photograph.
4. The fee is $50.00
5. Applicant’s old passport.
6. Photocopy of the old passport’s data page.

Passport Loss Requirements:

• Passport Application Form. Download **
• Copy of the lost passport if available.
• Police report.
• Three new photos 2×2
• $30 for a travel document.
• $25 Affidavit of Loss fee. Download **
